Dental Professionals

1. Pre-Anesthesia Appointment

Please contact our office to book our services on a day that is convenient for you. The pre-anesthesia appointment is then scheduled with the patient’s primary physician or pediatrician, to obtain what is called a "History and Physical" or "Wellness Check".  Please have the pediatrician/primary physician fax this report directly to our office at (844) 488-8242. Of note, be sure to have patients schedule the pre-anesthesia appointment to occur more than eight business days before the day of anesthesia.  This gives our clinical team sufficient time to review the patient’s  health history and address anything if needed.

2 Patient Intake Forms  (see link to digital and secure forms on "Patients" page)

3. Pre-Anesthesia Phone Call

One of our clinical nurses will call the patient (or patient’s parents) 7 days in advance of the dental anesthesia appointment. This will be a ten-minute phone call to review the patient’s medical history, go over what to expect on the day of treatment, answer any questions, and to go over payment arrangements.
